About quality measures and your personal health:

A quality measure is developed based on the current standard of good practice.  It is a general goal – an ideal fr the ideal patient.  Each physician should start with the ideal in mind. Then, the physician should think about you as an individual, what other health problems you have, the circumstances in your life that affect your health and might influence decisions and action, what your goals and values are, how you want to handle your health conditions – and decide with you what is clinically right for you to get you closer to “good health”.

You should do the same – the quality measure is an “ideal”.  You need to think about how your circumstances fit the ideal.  You should discuss your individual circumstances with your doctor.  Ask questions about how the “ideal goal” fits you.  How does it not fit?  Ask your physician/practitioner to explain in terms you understand - What is unique about your health and situation?  Why is this goal right for you?  Why is this goal not good for you?  How would the ideal goal be harmful to you?  How would it be good for you?  Ask – discuss  - Is there another goal you and your physician/provider can agree upon that would be a safe goal for you that still move you closer to “good health”.
